Divisions:

Men (Premier / Division2 / 18+ / 30+ / 40+) Women (Premier / 18+ / 30+) Co-Ed (18+ / 30+)

REGISTRATION:

Team Registration to be completed by Team Captain or Team Official Only.
Each Team must designate a Team Captain who is responsible for communications with SoccerFest Officials.
Each Team must have a minimum of 7 Players and a Maximum of 11 Players.
Each Player must fill out and sign the Player Waiver Form at www.winbigcharity.org/playerwaiver
Each Player must bring a copy of their ID with Photo to the SoccerFest.

IMPORTANT NOTES:

Team Captains or Team Officials will receive an email confirmation upon successful registration.
Players will receive an email confirmation upon successful waiver form submission.
Detailed tournament schedules will be shared closer to the Event date.

Each Player must be at least 18 years of age or older on the date of the Event.

All Team Captains registering a team must read, and sign this form by checking the box in order to participate in the Win Big Charity SoccerFest on October 18th 2025 at the Premier Sports Campus in Lakewood Ranch, Florida. Please note that Win Big Charity Inc. will NOT share any personal information - it will be confidential. You must be 18 years of age or older on the date of the event to participate in the Win Big Charity SoccerFest.

I AM AWARE of the potential health and safety risk and I ASSUME FULL RESPONSIBILITY for any personal injury, damage or death while participating in the Win Big Charity SoccerFest at the Premier Sports Campus in Lakewood Ranch, Florida. Such risk may include, but are not limited to, any injury or illness resulting from use, misuse and failure of any facility or equipment on site. Other risks of playing or participating any sport may include, but are not limited to, death, serious neck or spinal injuries which may result in complete or partial paralysis or brain damage, serious injury to any bone, joint, ligament, muscle, or tendon, and serious injury or impairment to other parts of the body, general health and well-being.

I ACCEPT the responsibility of discussing my participation in this event, including use of any services, with my physician and to obtain adequate medical, health, dental, accident, and all other forms of insurance coverage which may apply. I FURTHER ACCEPT the responsibility to inform myself on the use of any facility, equipment, or service during this event, and to seek assistance when necessary.

I UNDERTAKE to inform Win Big Charity Inc. representatives of any pain, discomfort or other symptom that I may experience during my participation in this event, or use of any facility or service during this event, and to discontinue such activity or use voluntary if required by Win Big Charity Inc.

I GIVE my permission for the free use of my image in broadcast, telecast or other media account of the event/activity and for the promotional purposes of the Win Big Charity SoccerFest.

IN CONSIDERATION of my participation in this event and the use of any facility or service during this event, I RELEASE Win Big Charity Inc., its directors, officers, employees, volunteers and other representatives from any and all liability, claim, demand or action for any present or future loss, damage, injury or expense that I may suffer or that my next of kin may suffer arising from such use or participation.

I ACCEPT that this document shall be binding upon my heirs, next of kin, executors, assignees, and representatives in the event of my death or incapacity. I HAVE READ, AND I UNDERSTAND ITS CONTENT, AND I ACCEPT that by signing this document, I am waiving certain legal rights, including the right to pursue legal action, which I or my heirs, next of kin, executors, assignees, and representatives may have against Win Big Charity Inc.
